Patients living within our practice boundary (please see below) are welcome to register.
When you register you will be allocated a named GP, however you are welcome to see any of the partners subject to appointment availability.
All new patients over the age of 18 must complete a new patient questionnaire and will be asked to have a new patient health check with the practice healthcare assistant.
When registering it is useful if you are able to provide your NHS number as this speeds up the registration process. If you are unsure what your NHS number is please speak to your current GP practice as they should be able to tell you.
You can download and print off a new patient pack using the link below or alternatively you can obtain a hardcopy welcome pack from one of our receptionists.
The new patient pack contains the following documents:
- GMS1 Registration Form
- New Patient Questionnaire
- "Welcome to our practice" letter
- Text Message Consent Form
- Information forms concerning the NHS Summary Care Record programme
- FAST Screening form
- Patient Participation Group application form
- Practice Information Booklet
For those under the age of 18 only the GMS1 form requires completion.
